Please print the new patient registration form and bring it to the office on your first visit. If you prefer to fill out your form in the office, please arrive 20 minutes early. Thank you.
Dr. Lin and Dr. Hua do not accept insurance, but you may personally submit your invoice for reimbursement to your managed care company if interested. Call them ahead of time to determine if you have any benefits towards acupuncture.
